What treatment at the clinic involves

AGA is a progressive form of hair loss common in men. Testosterone is converted by type II 5-alpha reductase into DHT (dihydrotestosterone), and DHT is understood to affect the hair growth cycle. How quickly it progresses, and where, differs from person to person, so the doctor first examines the condition of your scalp and hair.

Treatment here is with medication: tablets, a topical solution, or both. The consultation, including advice on daily habits, is free of charge, so what you pay for is the medication itself.

The medicines we prescribe

Combined hair-growth medication

This is the clinic’s own preparation, bringing minoxidil 5mg, finasteride 1mg and zinc together in a single tablet, at ¥13,800. Finasteride is known to suppress the production of DHT, which is associated with hair loss; minoxidil is understood to act on the blood supply to the scalp. Combining them reduces the number of tablets you take.

Single-agent prescriptions

Depending on your constitution and your own preference, starting with a single agent may suit you better. A finasteride generic made in Japan is ¥4,800 and topical minoxidil is ¥6,000. Which combination is appropriate is decided by the doctor at the consultation.

When results appear

Hair grows in a cycle of growth, regression and rest, so change takes time. One to two months after starting minoxidil there may be a temporary increase in shedding, known as initial shedding, which usually settles. Around the third month is a common point at which change is noticed, but this varies from person to person and the same result cannot be promised to everyone.

Your visit, step by step

• Book through LINE or the website. If there is a free slot we can often see you the same day.
• Complete the medical questionnaire when you arrive.
• The doctor examines your scalp and hair, and asks about your history and any medication you take.
• A treatment plan is proposed. Nothing is dispensed unless you agree to it.
• The medication is dispensed at the clinic and you pay for it. The consultation itself is free.
• If travel or a move makes attending difficult, you can switch to an online consultation.

Important information about this treatment

• Self-pay treatment: AGA treatment is not covered by public health insurance and the full cost is borne by the patient. No insurance card is needed, so no notice of medical expenses is sent to your home or your workplace.
• Unapproved pharmaceutical: oral minoxidil, including the minoxidil contained in the clinic’s combined hair-growth medication, is not approved in Japan as a pharmaceutical for the treatment of AGA. Finasteride and topical minoxidil are approved in Japan for male pattern hair loss.
• Adverse drug reaction relief system: where treatment uses an unapproved pharmaceutical, it falls outside Japan’s Adverse Drug Reaction Relief System, even in the event of a serious side effect.
• Main risks and side effects: shedding at the start of treatment, fluid retention, excess body hair, reduced libido, erectile dysfunction and impaired liver function. In a one-year clinical trial of finasteride conducted in Japan, side effects including abnormal laboratory values were seen in 4.0% of participants (11 of 276); the principal findings were reduced libido in 1.1% (3 cases) and erectile dysfunction in 0.7% (2 cases).
• Impaired liver function: rarely, loss of appetite and general fatigue may appear as signs of impaired liver function. If this occurs, stop taking the medication and speak to us.
• Blood donation: you cannot donate blood while taking these medications. Finasteride must be stopped for at least one month, and dutasteride for at least six months, beforehand.

How the effect appears and how long treatment is needed vary from person to person. Whether a medication can be prescribed is decided by the doctor at the consultation. Please ask the doctor about anything that is unclear.

Visiting the clinic

AGA treatment is something you keep going, so being able to get to the clinic easily matters as much as the treatment itself. The Shimbashi clinic is one minute on foot from the Ginza exit of JR Shimbashi Station, open until 21:00 on weekdays and from 10:00 to 18:00 at weekends and on public holidays — hours you can reach on the way home from work or on a day off. A repeat prescription helps you avoid missed doses and carries a standing discount, and you may pause treatment whenever you wish.

Getting to the clinic

Coming by other lines

Shimbashi is served by five lines. Toei Asakusa Line Shimbashi Station and the Yurikamome Shimbashi Station are both within walking distance, so the clinic is easy to reach from any part of Tokyo.

Coming from Shiodome, Ginza or Toranomon

The Shiodome office district is within walking distance. Ginza, Toranomon and Kasumigaseki are one or two stops from Shimbashi Station.

Coming after work

We are open until 21:00 on weekdays. First visits are accepted up to 30 minutes before closing and follow-up visits up to 10 minutes before closing. On Saturdays, Sundays and public holidays we are open from 10:00 to 18:00.

If you are hesitating

I am not sure my hair loss is far enough along to be treated.

There is no threshold you have to reach. The doctor looks at your scalp and hair and tells you what is happening; if treatment is not needed, we say so. Because AGA is progressive, coming early usually leaves more options open.

Will my family or my employer find out?

All treatment is self-pay and no insurance card is required. As public health insurance is not used, no notice of medical expenses is sent to your home or your workplace. The clinic treats men only.

I am not confident I can keep it up.

You can begin with the three-month trial at ¥18,000. With a repeat prescription you may pause treatment at any time and decide for yourself when to resume.

I would rather not take a strong medicine straight away.

Starting with a single agent, or with the topical solution alone, is possible. Tell the doctor what you are uneasy about and the plan can be built around it.

I do not speak much Japanese.

The consultation is conducted in Japanese. Please allow a little extra time, and bring a note of any medication you take. Written material helps the doctor confirm the details accurately.

Is there a consultation fee?

No. The consultation fee is ¥0, including advice on daily habits. What you pay for is the medication.

What medicines are available?

How long before I see a result?

This varies. Initial shedding caused by minoxidil usually settles within one to two months, and around the third month is a common point at which change is noticed. The way results appear differs from person to person.

What is the difference between the combined medication and a single agent?

Finasteride suppresses the production of DHT, which is associated with hair loss, and minoxidil acts on the blood supply to the scalp. The combined medication brings these together with zinc in one tablet, so you take fewer tablets. For some men, starting with a single agent is more suitable. The doctor decides at the consultation.

How does finasteride work?

It inhibits type II 5-alpha reductase, the enzyme that converts testosterone into DHT, the substance associated with hair loss. This is how it slows the progression of AGA and supports hair growth.

Can I donate blood while taking it?

No. You cannot donate blood while on these medications. Finasteride must be stopped for at least one month, and dutasteride for at least six months, before donating.

Is it a problem if my partner becomes pregnant while I am taking it?

No effect on sperm formation, semen production or testicular function has been observed with finasteride. In clinical trials, pregnancy rates among partners were similar between the finasteride group and the placebo group.
