Will daily Cialis (tadalafil) cause tolerance, addiction, or worse side effects? These concerns come up constantly among patients starting ED treatment. The short answer: Cialis does not produce pharmacologic tolerance or dependence, and the daily dose (2.5mg/5mg) actually tends to cause fewer side effects, as confirmed by clinical research. “Is it safe to keep taking Cialis every single day?” “Won’t I build up tolerance and stop responding to it?” These are some of the most common questions we hear from patients considering ED treatment with Cialis (active ingredient: tadalafil).
In fact, Cialis has an officially recognized once-daily low-dose regimen. Taking 2.5mg or 5mg every day keeps blood concentrations steady, allowing for spontaneous sexual activity without having to time each dose. The U.S. FDA approved this regimen in 2008, and in Japan tadalafil is also approved for the treatment of lower urinary tract symptoms associated with benign prostatic hyperplasia (BPH).
This article answers seven of the most frequently asked questions about taking Cialis daily, including tolerance, dependence, side effects, sexual function, ejaculation, and whether ED can ultimately be “cured”, all based on the best available medical evidence. Knowing the facts is the first step toward safe, confident treatment with Cialis.


“If I keep taking Cialis every day, won’t it stop working?” is one of the most common worries people raise when starting ED treatment. The straightforward answer: tadalafil does not cause pharmacologic tolerance.
Tolerance (tachyphylaxis) is the phenomenon in which a given dose of a drug becomes less effective with repeated use. It is well known with opioid analgesics and certain antihypertensives. However, tadalafil, a phosphodiesterase-5 (PDE5) inhibitor, works by inhibiting the PDE5 enzyme rather than binding to a receptor. Enzyme inhibition does not cause receptor downregulation (a drop in receptor numbers), so the drug’s effect is preserved with long-term use.
A two-year clinical study of daily tadalafil (Carson CC et al., 2006) found no significant decline in IIEF erectile function scores between baseline and the two-year follow-up; in fact, improvements tended to be sustained throughout the study.
While Cialis itself does not cause tolerance, some patients do report that “it doesn’t seem to work as well as it used to.” Three causes are usually behind this:
If the drug feels less effective, do not increase the dose on your own. Talk to your doctor about adjusting the dose or switching to another ED medication such as Viagra or Levitra.


“If I take Cialis every day, will I lose the ability to have erections without it?” is another common concern. Cialis does not cause physical (substance) dependence.
True dependence occurs with drugs that act on the central nervous system, such as alcohol, benzodiazepine sleep aids, and opioids. Cialis acts on peripheral blood vessels (the smooth muscle of the corpus cavernosum), and has no effect on the brain’s reward system or dopaminergic pathways. Discontinuing it does not cause withdrawal symptoms.
If you stop taking Cialis, your erectile function simply returns to its pre-treatment baseline. The drug itself does not “make ED worse.”
Although there is no physical dependence, some patients develop psychological anxiety, fearing that “sex won’t work without the pill.” This is called psychological dependence and is especially common in patients with psychogenic ED.

“Won’t side effects build up and become worse if I take it every day?” is another frequent worry. In reality, daily dosing tends to produce milder side effects than on-demand use.
The reason is simple: each individual dose is smaller. Compared to 10-20mg taken on demand, daily dosing uses just 2.5-5mg – one quarter to one half the dose. The sharp blood-level peak is blunted, so adverse effects are gentler.
Common side effects of Cialis and their reported incidence (from the prescribing information):
Clinically, most of these effects diminish within a few days to two weeks as the body adjusts. However, if you experience visual disturbances or a prolonged erection lasting more than 4 hours (priapism), stop taking the drug immediately and seek medical care.

Some patients worry that “if I keep taking ED medication, my own natural erectile capacity will get even worse.” However, there is no medical evidence that long-term Cialis use weakens sexual function.
On the contrary, daily-dose Cialis has been linked to possible improvement of vascular endothelial function. Cialis does not change the underlying mechanism of erection. It simply works by inhibiting PDE5 and slowing the breakdown of cGMP (cyclic guanosine monophosphate). It does not impair natural erectile function.
“Will long-term Cialis use lower my testosterone?” is another common concern. There is no evidence that PDE5 inhibitors influence testosterone production or secretion.
Cialis acts only on peripheral blood vessels and does not interact with the hypothalamic-pituitary-gonadal (HPG) axis. If late-onset hypogonadism (LOH-related ED) is suspected, blood tests can measure hormone levels and testosterone replacement therapy can be considered as an adjunct if appropriate.


You may sometimes hear comments like “Cialis makes it hard to reach orgasm” or “Ejaculation takes longer once I started taking it.”
Cialis has no pharmacological action that directly delays ejaculation. Tadalafil acts on the PDE5 enzyme and does not affect the sympathetic nerves or serotonin receptors involved in ejaculation.
However, because the erection is firmer and better maintained on Cialis, some men notice the following:
For men with premature ejaculation, this may actually be a benefit. If, on the other hand, ejaculation feels excessively delayed, ask your doctor about adjusting the dose.


Cialis is an ED medication, not a cure for the underlying condition. PDE5 inhibitors are symptomatic treatments that support erection. The benefit ends when you stop taking the drug.
